- Original Birth/Marriage/Death Certificate authenticated by the Consular Division of the Ministry of External Affairs in Sri Lanka
- Money order for C$ 40.00 in favour of “Consulate General of Sri Lanka” or Cash if submitted at the Consulate counter.
- Prepaid envelope or additional C$ 16.00 as postage
*Please note that, no certificate will be translated, if the document is not authenticated by the Ministry of External Affairs in Sri Lanka
Note1. This High Commission is not responsible for any document lost in the post.
2. Submission of forged documents and making false declarations is a punishable offence and the fees already paid will not be refunded.
3. Incomplete applications will be rejected.
