 Valid passport (Sri Lankan or Canadian (**Canadian Emergency Passport not accepted)

1. Civil Status Report (Obtain from Service Ontario and needs to be certified by the Ministry of Government and Consumer Services at 222 Javis Street Toronto)
2. Self-Declaration for Health Condition (Written Letter disclosing Health and Vaccination status)
3. Police Report for “No Criminal record” (Obtain from Police H.Q at 40 College St;or online)
4. Original Birth Certificate (No Translations accepted)

 Witnesses: You need to bring 2 witnesses with valid Sri Lankan Passports and valid Canadian PR Cards to sign as witnesses in the presence of the Consulate officer.

Witnesses needs to provide a copy of the Passport and PR card

-          Proceed to the Ministry of Government and Consumer Services at 222 Javis Street Toronto, and have the POA  notarized.

-          Once notarized make a copy of the entire document.

Certificate for 2^nd Bank Account Opening

-          Kindly be informed that you will have to complete any forms given by the respective bank and adhere to the following 

Sri Lankan Citizens with Valid Sri Lankan Passport

1. Duly completed bank forms (if any)

2. Original Sri Lankan Passport

3. Permanent Resident Card/ Canadian Status document

4. Make a set of all original documents and ID’s

5. On completion visit the Consulate Monday to Friday between the hours of 9.30a.m to 01.00 p.m.

6. Applicable Consular Fees: $ 60.00 payable only in cash.

Canadian Citizens with valid Canadian Passport.

1. Duly completed bank forms (if any)

2. Attach a copy of the Canadian Citizenship

3. Copy of Canadian passport

4. Go to a practicing Canadian Lawyer and get all documents certified and an official stamp affixed.

5. Once done place all documents in one bundle and go to the Ministry of Government and Consumer Services at 222 Jarvis Street Toronto, and get the bundle certified as one document.

6. On completion visit the Consulate Monday to Friday between the hours of 9.30a.m. to 01.00 p.m.

7. Applicable Consular Fees: $ 65.00 payable only in cash.
